The HRQ is available year-round. You can access and return your HRQ in one of three ways:

• Online: www.takeyourHRQ.com
• Phone: Call HMC (the HRQ provider) toll-free at 888-901-0477
• Paper: Contact your Union Local, the Fund Office, or HMC to request a paper HRQ

HMC automatically reports this activity to the Fund Office. You don’t have to return a form.

• Gold/Silver Participants: The Fund contributes $125 to your HRA.
• Platinum Plus/Platinum Participants: The Fund contributes $150 to your HRA.

Remember, you can earn up to the maximum allowed in one year.
