A health screening can help you identify potential risks that lead to illness and give you ideas on how to live a healthier life.
Your screening will include the following health-related tests and measurements. Your individual results are confidential; they will NOT be shared with your Employer or Union.

You can get a screening through your doctor or a participating in-store pharmacy at no cost to you. (However, if your doctor coordinates your screening, to ensure 100% coverage, you need to visit an in-network provider and ask the office to code your visit and lab work as “preventive.”)
Getting your screening through your doctor. Contact your doctor to schedule your screening (ideally, do this with your annual physical). To get credit for this Healthy Activity, you will need to return the Physical Exam and Health Screening Form to the Fund Office. Provide the form to your doctor and ask him/her to complete it. Then, it’s your responsibility to return the form to the Fund Office.
Getting your screening through a participating in-store pharmacy. You’ll need to find a pharmacy and schedule your appointment. Once complete, reporting to the Fund is automatic.

A form is ONLY required if you get your screening from your doctor. You’ll need to take the Physical Exam and Health Screening Form to your doctor. You can get a copy of the form here or call the Fund Office to request one.
