Get Preventive Health Care Services

You become eligible to receive HRA dollars when you get the following preventive care services (appropriate for your age and gender):

  • Annual flu shot
  • Colonoscopy
  • PSA test (for men)
  • Mammogram (for women)
  • Pap Smear (for women)

In most cases, your service(s) are processed through the Fund’s claims system. This should trigger your HRA contribution. However, sometimes a Preventive Health Care Services Form may be required.

These preventive health care services are available at no cost to you — meaning, there is no deductible to meet and the plan pays 100%. Just make sure to visit an in-network provider and confirm that your provider codes your visit(s) as "preventive."

The Fund contributes the following to your HRA for each preventive care service. But, remember: you can only earn up to the maximum allowed each year.

  • Gold/Silver Participants: The Fund contributes $125 to your HRA.
  • Platinum Plus/Platinum Participants: The Fund contributes $150 to your HRA.

In most cases, when you get your services in-network, they are processed through the Fund’s claims system. However, if you receive services from a clinic or other facility (e.g., your husband gets his flu shot at work), return the Preventive Health Care Services Form to the Fund Office.  You can get a copy of the form here or call the Fund Office to request one.
